Tests ordered through the labs are generally self-pay (out-of-pocket). The company does not bill insurance or Medicare directly, which is part of how we keep pricing lower.  If you choose, you may submit your receipt and associated CPT codes on your own to your insurance company for possible reimbursement; however, coverage decisions depend entirely on your individual plan.  Self-ordered tests are usually not covered since most insurers will only review claims that include a physician-ordered and physician-provided diagnostic (ICD-10) code.  You can contact your insurance company in advance to ask whether any portion of the test(s) may be eligible for reimbursement under your specific plan.
