What is the CIRS Test?

The CIRS test is a lab panel that helps you find parts of your immune system that might be causing Chronic Inflammatory Response Syndrome (CIRS). CIRS is usually linked to biotoxin exposure. Biotoxins are

toxic substances made by different organisms. In people, these are often molds or long-term infections, which may not cause obvious symptoms.

To help confirm mold exposure, we recommend testing for mycotoxins.

Dr. Shoemaker and CIRS

Dr. Shoemaker is one of the top experts in CIRS research. He recommends several lab tests that give helpful clues about which parts of the body are being affected by the syndrome.

Below are the key CIRS lab tests included in one panel. Contact us if you need other CIRS-related tests.

Markers in Your Chronic Inflammatory Response Syndrome Test Order

Marker Name Marker Name
MMP9 ADH
TGFb-1 MSH
VEGF
Leptin

What does each of the markers mean?

MMP9

MMP9 is made by certain white blood cells. It breaks down blood vessel walls so other substances can enter tissues like the brain, muscles, and joints. In CIRS, the body makes too much MMP9, letting in lots of inflammatory substances that cause inflammation all over the body.

C3a and C4a (add-on)

The C3a and C4a tests measure specific proteins in your blood that are part of your immune system’s response to threats like infections or inflammation. Elevated levels of these proteins can signal that your body is reacting to something harmful, such as mold exposure or chronic infections. Doctors sometimes use these tests to help understand unexplained symptoms like fatigue, brain fog, or body aches.

VIP (add-on)

The VIP (Vasoactive Intestinal Peptide) lab test measures a hormone that helps regulate blood flow, digestion, and immune response. Low VIP levels are often found in people exposed to mold or dealing with chronic inflammatory conditions. This test can help doctors figure out if your symptoms—like dizziness, digestive issues, or brain fog—might be linked to VIP imbalance.

ADH

Anti-diuretic hormone (ADH), also called vasopressin, helps your kidneys keep water in your body. This prevents dehydration and helps balance electrolytes (osmolality). In mold-related CIRS, people might pee a lot, feel really thirsty, and get dehydrated.

When electrolytes get off balance in CIRS, nerve signals can change. This can cause ongoing pain, which can trigger even more inflammation.

Leptin

Leptin is a hormone made by fat cells that tells your brain you’re full. But leptin also increases how much fat gets stored. In CIRS, inflammation blocks leptin signals to the brain, which makes you feel hungrier and can lead to weight gain.

MSH

MSH stands for Melanocyte Stimulating Hormone. It’s made by the pituitary gland and helps control inflammation. It also helps fight germs like mold, which makes the biotoxins that cause CIRS.

Leptin helps the body make MSH. When MSH is low, it can make things like mold sensitivity, tiredness, pain, trouble sleeping, and hormone issues worse.

VEGF

VEGF (Vascular Endothelial Growth Factor) helps new blood vessels grow in low-oxygen areas. Long-term inflammation lowers VEGF, which means less oxygen in your tissues. This can cause muscle cramps, feeling very tired after exercise, and even shortness of breath.

TGFb-1

TGFb-1 can increase or decrease inflammation. High TGFb-1 levels happen when the immune system is too active. It can also harm other white blood cells that protect you from autoimmune problems, raising your risk.

Frequently Asked Questions for the Chronic Inflammatory Response Syndrome Test

What is Chronic Inflammatory Response Syndrome (CIRS)?

CIRS is a complex, multi-symptom illness triggered by exposure to biotoxins such as mold, bacteria, or other pathogens. It often affects individuals with a genetic predisposition and can lead to a chronic inflammatory response.

Why should I take this lab test?

This test is essential for managing Chronic Inflammatory Response Syndrome. It helps identify specific biomarkers that indicate the presence and severity of CIRS.

How do I prepare for the lab test?

No special preparation is required, however, it’s advised to consult with your healthcare provider regarding any medications or supplements you are taking, as may have the potential to affect the test results. If you add on the Vasoactive Intestinal Polypeptide (VIP), a 12 hour fast is needed.

How is the CIRS lab test performed?

The CIRS lab test is performed through a simple blood draw, which is then sent to the laboratory for testing.

How long does it take to get the test results?

Due to the complexity of testing, results from the CIRS test are usually available within 20-22 business days. Your results will by posted under My Account → Test Results. You will also receive an email notification when labs are completed.

What do my test results mean?

Test results will show whether certain biomarkers are elevated or within normal ranges. Abnormal levels can suggest the presence of CIRS, and your healthcare provider will interpret these results in the context of your symptoms and health history.

Can CIRS be treated?

Yes, it can be managed through a combination of therapies, including addressing the source of biotoxin exposure, detoxification, and immune system support. Including a functional medicine practitioner in your treatment is extremely helpful.
