Collection restrictions
The test(s) you have chosen cannot be collected in NY, NJ, MA, RI or outside the US.
- Even though NY, NJ, Mass., and RI locations will show on the locator tool, samples can NOT be drawn within their borders.

Requisition form
Your requisition form is emailed to you shortly after purchase. Print out your form and take it to the draw center. Appointments are encouraged but is necessary.

Draw fee
Draw fee is included with your purchase!

Not for test kits
These draw centers will not draw for test kits from our other lab processors (e.g. Diagnostechs Labs, Genova Labs, Cyrex Labs, etc.).
