Home Lab Test Kit Instructions
- The lab test kit will be mailed to your shipping address.
- If the requisition form is not in the kit, it will be emailed to you. Contact us if you do not receive it.
- Sample collection can be completed at home. Follow the collection instructions carefully.
- Unless noted in the test description, home test kits will contain all necessary return mailers in which you can schedule a pickup or drop off (Monday-Thursday only).
- Contact us with any questions. Do not contact the lab directly.
