• Test kits are mailed to your shipping address.
• Your requisition form will be emailed to you. 
  • Schedule your draw here once you have received your emailed requisition form.
  • If no draw centers are in your area, you may contact any local clinic or hospital to complete the draw (draw fees may apply).
  • Pediatric (15 years old and younger) sample collections are not included in Cyrex’s contracted draw centers. Contact a local clinic or hospital for collection. Draw fees may apply.
• Use the free shipping return mailer found in the kit.
